چکیده

consecutive procedures of pulmonary vein isolation were performed for treatment of paroxysmal and persistent atrial fibrillation. These interventions constitute the initial experience of the Instituto Cardiovascular de Buenos Aires, where 260 procedures are currently performed each year. Our population consisted mainly of men with an average age of 55.58 years; 71.9% had no history of cardiovascular disease. Paroxysmal atrial fibrillation occurred in 65.3% of cases over an average of 7 years. All patients had received three different antiarrhythmic agents excluding beta blockers or calcium channel antagonists. The average number of episodes of atrial fibrillation was 6 per year. The success rate during the procedure was 97.87%. The primary success rate at 12 months was 84.04%, 88.32% for the group without heart disease and 64.8% for the group with heart disease. Mean follow-up was 18±6 months. In our experience, pulmonary vein isolation is a treatment strategy for very selective cases with an acceptable primary success rate.

Pulmonary Vein Isolation for Treatment of Paroxysmal Atrial Fibrillation on Patient with Situs Inversus Totalis.

A 56-year-old male with paroxysmal atrial fibrillation refractory to class IC and class III antiarrhythmic drugs was admitted to our hospital for radiofrequency catheter ablation of atrial fibrillation. During preoperative examination situs inversus totalis was revealed. Pulmonary vein (PV) isolation was successfully performed with atrial fibrillation termination and elimination of all PV poten...

Durable Pulmonary Vein Isolation: The Holy Grail of Atrial Fibrillation Ablation.

The inability to achieve durable pulmonary vein isolation remains a major limitation to catheter ablation for the treatment of atrial fibrillation (AF). In this review, we discuss the research performed over the past decade investigating methods to improve lesion permanence for the goal of durable pulmonary vein isolation (PVI). Investigations evaluating procedural techniques, adjunctive pharma...

Complete pulmonary vein isolation using balloon cryoablation in patients with paroxysmal atrial fibrillation.

Electrical isolation of the pulmonary veins and disconnection of the left atrial musculature from their arrhythmogenic effects is the cornerstone of definitive and curative treatment in patients with symptomatic recurrent paroxysmal atrial fibrillation that is refractory to antiarrhythmic therapy. The type of lesion produced by balloon cryoablation is such that the tissue architecture is preser...

Percutaneous Modalities for Atrial Fibrillation Ablation

Pulmonary vein isolation (PVI) is now accepted as a cornerstone modality for the treatment of paroxysmal atrial fibrillation (PAF) ablation (1). The technique has been started with focal pulmonary vein (PV) ablation of AF triggers or ostial isolation of the PVs (2-5). Nowadays, wide circumferential ablation of the PV antrum is the preferred approach (6). The potential role of different non-PV t...

Thrombus Development on a Transseptal Sheath in the Right Atrium Before Electrical Pulmonary Vein Isolation

We describe the case of a patient who developed a thrombus on the transseptal sheath in the right atrium before transseptal puncture for circumferential pulmonary vein isolation for paroxysmal atrial fibrillation treatment. The use of intracardiac echocardiography allowed to its identification and probably prevented the patient from suffering a serious thromboembolic complication.

Video-assisted bilateral pulmonary vein isolation and left atrial appendage exclusion for atrial fibrillation.

OBJECTIVE Pulmonary vein isolation is curative in selected patients with atrial fibrillation. The objective of this study was to assess the feasibility and safety (midterm results) of video-assisted thoracoscopic epicardial pulmonary vein isolation. METHODS Twenty-seven patients (22 male patients) with atrial fibrillation (18 paroxysmal, 4 persistent, and 5 permanent; average age, 57 years) u...
